A hallucination is when an AI model produces confident but incorrect or invented information.

It happens because language models generate plausible text rather than looking facts up. Grounding answers in your data, requiring citations and adding checks greatly reduce it.

For analytics, ground the model in a semantic layer so it cannot invent how a metric is calculated.
